DD Form 2569 completion/submission is mandatory for all retirees and 
dependents that use or will use the Fox Health Clinic. 

Those retiring soon will need it also.  Separate form required for both sponsor and spouse.

Turn-in the completed DD Form(s) 2569 at the Medical Records counter at Fox Health Clinic.

Each person will be issued another card that must be shown at time of visit to the Clinic or Pharmacy.
